Channels decay by default into empty rituals while appearing functional from the outside, so managers must continuously audit whether each recurring structure still surfaces new, true information. Psychological safety is a load-bearing property of any channel: a single instance of punishing honest feedback kills the channel while it continues to look alive. Different information types need different channel shapes (fast and narrow for incidents, slow and wide for architectural disagreement). The org chart itself is the most consequential channel-design decision, silently determining which conversations are cheap or expensive before any meeting is scheduled.
