# How Claude Code Handles Code Reviews at AI Scale

## Summary

The Claude Code team rebuilt code review into a 7-layer continuous validation system to keep up with AI-generated code. With AI writing nearly 90% of Claude Code's codebase, daily merged changes jumped from 2,500 in May to over 30,000 in September. The layers include pre-PR verification by Claude itself, multi-agent AI review with a verifier panel, automated security and CI checks, risk-based approval tiers, and safe rollouts with automated incident evals. By shifting routine reading to machines and saving human review for architecture and product decisions, the team doubled merged pull requests while cutting revert rates by over 40%.

I was reading about how the Claude Code team handles Code Reviews at AI scale, and I found it fascinating. With AI generating nearly 90% of Claude Code’s codebase, traditional line-by-line manual code reviews quickly reached a bottleneck. Across Anthropic, their daily merged changes jumped from 2,500 in May to over 30,000 in September.

To maintain quality without slowing down, the Claude Code engineering team reshaped code review into a **7-layer continuous validation system**:

1. **_Pre-PR Verification_**: Before a pull request is even opened, Claude runs test suites, builds, and linters, while a second subagent inspects the diff in a fresh context.

1. **_Multi-Agent AI Review_**: Parallel AI agents analyze the PR against the entire codebase. A separate panel of verifier agents votes on findings to filter out noise before posting prioritized inline comments.

1. **_Automated Security & CI_**: Automated security scans and continuous integration checks run in parallel. Unresolved security or bug findings block the merge.

1. **_Risk-Based Approvals_**: A classifier categorizes changes into risk tiers. Low-risk modifications (like documentation or flagged features) can merge automatically once checks pass, whereas high-risk code paths strictly require human approval.

1. **_Safe Rollouts & Incident Evals_**: Code lands behind feature flags with merge queues and fast rollback mechanisms. Whenever a bug or revert occurs, it is converted into an automated eval case to continuously benchmark and refine the AI reviewer.

By shifting routine code reading to machines and reserving human scrutiny for high-leverage decisions - like architecture, taste, and product direction - the team doubled merged pull requests while reducing revert rates by over 40%.

BTW, a well-built AI software factory can help solve many engineering bottlenecks. I recently built a small one myself, and you can see how it works here: https://youtu.be/BiTqVaL6R80

> At 30,000 merges a day, what share actually lands in the human-approval tier? And does that number go up or down as the reviewers improve?

> The risk-tier question in the comments is the one that decides whether this scales. A useful metric is the share of auto-merged changes that later needed a revert, since that shows if the classifier is too loose. For a smaller team, starting with only documentation and config changes in the auto-merge tier is a safe first step.

> The idea of a second agent inspecting the diff in a fresh context before the PR is even opened stands out, since it avoids the author agent grading its own work. Having verifier agents vote on findings to filter noise also addresses the usual complaint that automated review is too chatty. A practical question is how teams calibrate the risk tiers so low-risk auto-merges don't quietly drift into riskier changes over time.

> There's one failure mode I'd be careful about with the multi-agent voting approach: correlated mistakes.

A PR adds an authorization check that verifies the user is authenticated, but forgets to verify that the requested resource belongs to that user’s tenant. Multiple reviewers might accept the implementation because the authentication flow looks correct in isolation.\nI’d like to see an independent validation layer that doesn’t just ask another agent to review the same diff. For example, generate cross-tenant access test cases, mutate or remove the ownership check, and verify that the test suite actually catches the regression.\nThat distinction matters: reviewers evaluate the implementation, but tests need to challenge the assumptions behind it.\nAt this scale, I’d be more interested in measuring whether the system can consistently catch deliberately introduced bugs than in measuring how many agents agree on a review.","datePublished":"2026-10-31T12:11:19.829Z","url":"https://daily.dev/posts/NuSyCUVVc#c-bINYhNPZs","author":{"@type":"Person","name":"Sina Rezaei","url":"https://daily.dev/sinarezaei","image":"https://lh3.googleusercontent.com/a/ACg8ocLXjJ5mCeyMCQK6CSjPfPxZisLuVwj3KOgnAtXoJoPAAsuvSrxh=s96-c"}}],"isPartOf":{"@type":"WebPage","url":"https://daily.dev/sources/dfa0u7-fs","name":"Pavan Belagatti"}}
